1)  NEW MEMBER APPLICATION - "ON-LINE" (Complete online and submit) DUES and WAIVER will need to be mailed/delivered to the name and address listed at the top of the online application.

2) NEW MEMBER APPLICATION for "PRINTING" (Print PDF document and mail/deliver to the name and address listed on the form. Dues payment and Tennis Facilities Waiver also required to activate membership.

3) QC TENNIS FACILITIES WAIVER FORM (Required of all current and new members) Form cannot be submitted online.

Print Form (PDF document), SIGN and deliver to any board member.

 

Club membership is not active until dues and all required paperwork is received by the Club.
